Florida divorce petition form

This is the official Florida Family Law Rules of Procedure Form 12.901(a), used by a husband and wife who both agree to end their marriage without a trial. It applies only when there are no minor or dependent children, no pregnancy, no request for alimony, and both spouses have already agreed on how to split property and debts. Using this form lets couples avoid a regular contested divorce filing and go through a simpler court process instead.
