SS-4 form

Form SS-4 is the official IRS application used to request an Employer Identification Number (EIN). Businesses, trusts, estates, churches, government agencies and other entities use it to register with the IRS for tax purposes. It asks for details such as the legal name of the entity, mailing address, responsible party and the type of entity applying, so the IRS can assign a unique tax ID.
